Adult Nursing Services

Adult Nursing Services are planned to meet a variety of health conditions experienced by adults receiving specific services on the DD Waiver. These services are intended to complement, not replace, medical care provided by an individual’s physician(s) or other licensed health care professionals and are designed to support the highest practicable level of health, functioning and independence for individuals, age twenty-one (21) and older.  These services are an option to those who reside in a Family Living setting; receive Customized In-Home Supports; or participate only in Customized Community Supports and/or Community Integrated Employment without accessing any Living Supports or Customized In-Home Supports.

 

There are two categories of adult nursing:

 

• Nursing Assessment and Consultation – The nurse completes an initial and annual comprehensive health assessment, analyzes the results of the assessment and consults with the individual, guardian and team regarding the findings and any recommendations that may include Ongoing Nursing Services.

• Ongoing Nursing Services – The nurse provides focused nursing supports based on the individuals needs identified in the individual’s comprehensive health assessment.  These services may include: Health Care Planning and Coordination, Aspiration Risk Management, Delegation of Nursing Duties, Medication Oversight, Medication Administration, and Coordination of Complex Conditions.

 

Trained registered nurses who are sensitive to the unique health challenges that may be present provide these valuable Nursing Services.
